Membership enrollment/renewal

A new enrollment form must be filled out and signed by each member every year. It is very important that you print legibly all the entries. When you print your name, it is important to use the same name and spelling that is on your National HOG card. If you have a nickname that you like to use, you may add it in parenthesis. Be sure that your e-mail address, if you have one, is very clear. We will be using e-mail frequently this year to communicate up-to-date information to our members.

We encourage all members with Internet access to check our Chapter web site at least once a week for updates and announcements.

If you choose to send your application with payment to the dealership, please mail the completed and signed form with your payment to Gene Lummus Harley-Davidson, Inc., 20 Patton Cove Road, Swannanoa, NC 28778 (Attn: Smoky Mountain HOG Chapter). You may bring your completed form to the Re-Up meeting/party in January. See the Chapter events page for more information about the meeting.

To download our Chapter enrollment form, click here. The form is one page in Adobe Portable Document Format (.pdf file extension). You will need Adobe Acrobat Reader installed on your computer. Adobe Acrobat Reader is free from Adobe. You may from Adobe if you don't already have it on your computer. Be patient with Acrobat Reader download, it's a very large file. Installation instructions are available here. If you have questions about installing the program, feel free to send our Chapter Web Master an e-mail asking for help. Chapter newsletters on our web site are also be in Adobe PDF format as well.

For our members that do not have or don't want to download the Acrobat Reader, the Chapter enrollment form is also available as a .rtf document that can be read by such applications as MicroSoft Word or Works. Click here for the .rtf version of the form.
